Statutory Declaration of Age

This is also called affidavit as to age declaration.

If the person was born but his age was not officially registered then this affidavit is for you. A person who don’t have birthday certificate can automatically use affidavit of age declaration. It serves same purpose as birthday certificate.

Requirements.

• Parents full name, address, gender, signature.
• Son/Daughters full name, date of birth, place of birth.
• Email address.
