Affidavit as to change/Correction of name

Affidavit of change of name is for those who wishes to change their name mostly for married couples or anyone that wishes to correct their name.

We will prepare the affidavit and take it to court for Registration and signing. Once this process is complete, the affidavit will be scanned to you via email for those not resident in our location.

Requirements

• Full name, Gender,  Signature, address, former name, current name.
• Email address.
